How to Add a Contact Form to a Non Product Page

Please note that only one Contact Form can be added to each web page

1. Log in as Website Admin

2. Select Settings and Edit Site Content from the menu bar

3. To add a new Content Widget to a Non Product page, navigate to the page you wish to add content to e.g Contact Us and select Add New Content

4. Select your preferred Widget Layout from the Standard Tab - you can select from the following content blocks. For example if you want to display a Contact Form and have some text beside the Contact Form you would choose a Two Content Block, if you just want to display a Contact Form then One Content Block would suffice

• One Content Block
• Two Content Blocks
• Three Content Blocks
• Four Content Blocks
• Advanced Layouts

5. Once a preferred layout is chosen, a Widget Manager will appear to help you develop and manage your content.

6. To Add a Contact Form, Choose Contact Form from the drop down for the relevant Widget.

7. Once Contact Form is selected additional fields will appear

• Reference*- add a reference for the Contact Form e.g General Enquiries or Xmas2019 Competition
• Send to email - If you would like the response to this email to go to an alternative email address (i.e in addition to the email address already in use for your orders) please enter the additional email address here 
• Custom Comments Title - If you would like to display a comments box enter the title here and the field will appear e.g. Your Comments, Ask your Questions Here etc
• Custom Textbox 1(2,3) Title - If you wish to add additional text boxes above the Custom Comments field please enter the titles here and the fields will be added. Note that you can add up to 3 custom textboxes. E.g Name of Doctor, Any Allergies etc
• Custom Dropdown Title & Value - You can ask a question and give optional answers using the Custom Dropdown Title & Value e.g Custom Dropdown Title : Is this a new prescription , Custom DropDown Value : New,Repeat. Once you add the title and values the fields will appear. With the custom values it is important that they are a comma separated list with no spaces. If using Location as your Drop Down Title then your values should be Ennis,Lahinch,Kilkee . It is also possible to have an email associated with the Location value so that if their location is picked the email is sent to them eg Ennis=ennis@magico.com,Kilkee=kilkee@magico.com
• Custom File Upload- It is possible to provide the option for the user to upload a file that is attached to the email that is sent to the Retailer. Provide a title for the Custom File Upload and the field will appear e.g. New Prescription and this will prompt the user to upload their prescription. A file can be an image or a document.    *denotes mandatory fields     

8. Complete the following Widget Container Options

• Container Title: If you want to add a Container Title explaining what this content is about insert it here e.g Your Prescription Request, Your Wedding Details etc
• Title url: if you want to link from the Container Title to a webpage insert the url here. Please note if you are linking to an internal page you should use a relative link
• Is Active*: If you want the page to appear live on the site ensure it is set to Active *denotes compulsory fields

10. To view the changes before they go live, select Preview

11. Save your changes and they will appear live on your site
